Fråga: Can Cipramil (citalopram) or Risperdal (risperidone) cause alopecia areata? Background: A 53-year-old woman with psychosis has been treated with Cipramil (citalopram) 80 mg daily and Risperdal (risperidone) 4 mg daily for the last six months. After one month of treatment she began to develop alopecia areata. No concomitant drugs have been used.

Sammanfattning: Although alopecia areata has not been described as an adverse effect of citalopram or risperidone treatment in the literature, the association between hair loss and treatment with citalopram or risperidone is supported by a number of case reports.

Svar: Similar questions have previously been dealt with in Drugline. No information concerning alopecia during citalopram or risperidone treatment has been found in the literature, although this side effect has been described for other selective serotonin reuptake inhibitors (SSRIs) as well as for other antipsychotic agents (1,2). An updated extensive literature search in Drugline, Medline and common pharmacological handbooks has not revealed any further information.

Swedis, the Swedish Adverse Drug Reactions Advisory Committee´s (SADRAC) database contains one report concerning defluvium where risperidone was judged as a possible cause of the symptom (3). In Intdis, WHO´s adverse reactions database, there are nine reports of alopecia associated with risperidone. However, these reports are not evaluated due to the differences in the reporting systems in the participating countries (4).

Other SSRIs, eg fluoxetine, have been associated with alopecia (2). According to the manufacturer there have also been reports on citalopram. Since 1998 they have received two reports of alopecia and one of hair loss after citalopram treatment (5).

The files of SADRAC contain three reports of defluvium and one of hair disorder, possibly caused by citalopram (3). In Intdis there is a total of 322 reports of alopecia associated with SSRIs. Twenty-two of these are after citalopram treatment (4).

We recommend this case be reported to SADRAC.
