Sammanfattning: "B-vitamins in therapeutic doses are considered as safe during lactation. However, the content of B-vitamins in B-Kombin Forte is higher than the recommended dietary allowance, and a change to ""ordinary"" B-Kombin tablets is recommended."

Svar: B-Kombin Forte is a combined vitamin B preparation containing thiamine (15 mg), pyridoxine (5 mg), pantothenic acid (25 mg), nicotineamide (125 mg), and riboflavine (15 mg) (1). These vitamins are naturally present in breast milk and maternal supplementation in therapeutic doses during breast-feeding is considered as safe (2,3). No studies showing negative effects on the breast-fed infant were found in the literature. Very large doses of pyridoxine (600 mg/day) have been reported to inhibit lactation (2,4), but later studies have failed to confirm these findings (4). On the other hand, pyridoxine deficiency has been identified as causing seizures in newborn infants (5). In Sweden, the National food administration recommends a daily intake of about 1.5 mg of each of the vitamins in B-Kombin Forte for nursing women (6). The American Society of Health-System has estimated a similar recommended dietary allowance of vitamins and minerals during lactation for the first six months (2). As related to these recommendations, the content of B-vitamins in B-Kombin Forte is too high, and hence a change to "ordinary" B-Kombin tablets is recommended.
