Sammanfattning: There is no relevant data concerning ocular side effects of haloperidol as monotherapy in the literature. A prospective study of patients under haloperidol monotherapy would be important to state whether ocular side effects occur.
Svar: Lens opacities as well as other ocular side effects have been reported in patients treated with phenothiazines (1) but the evidence for haloperidol to cause such effects is not at all convincing. Nevertheless there are some reports in Japanese journals suggesting butyrophenone derivatives as a possible cause of cataract, opacities of the cornea and lens and pigmentation (2). In a review (3) it is suggested that blurred vision found by other authors in 40 out of 250 treated patients could be due in fact to the antiparkinsonian drug benzotropine metanesulfonate that was given simultaneously instead of being caused by an anticholinergic effect of haloperidol itself.
A search through Medline and the Swedish Drug Information System was done with the following findings:
Swedis has in its files just one report about suspected haloperidol caused retinopathy in one patient but the data is too scarce to drawn any conclusion.
A Medline search covering Jan 77 to Feb 83 found just one paper from a Japanese journal about one case of a malignant syndrome with persistent nystagmus and diplopia but the patient was treated with lithium and methotrimeprazime besides haloperidol.
